We provide FD-258 fingerprint cards. If you're not getting cards, but instead getting an electronic or Direct Submission, please bring any and all forms required for your situation. If sent by your employer with a live scan direct submission form, bring an additional copy if your employer needs one for their records. We charge $1.00 for each page you have us print.
